To report a student's absence, tardy, or to request a college visitation day, please use the form linked below. This single form now combines all three functions for your convenience.

For Absences:

Please submit the form within three school days of the student's absence to excuse them due to illness. This will eliminate the need for you to call. For students absent from school for four or more consecutive days, a doctor's note is required.

For Tardies:

Please fill out and submit the form the day of the student's tardy to school. This will eliminate the need for you to call. A parent or guardian note for being late to school due to illness will be accepted up to five times per semester. The sixth and any subsequent tardies due to illness will require a valid doctor's note. If no note is provided, the student will be credited with an unexcused tardy.

Only tardy excuses from a doctor, dentist, a parent or guardian in relation to illness or a religious holiday, or a bus driver for a late school bus will be considered a legal tardiness. In extreme emergencies, a direct phone call on the day of the tardiness from a parent or guardian to the appropriate administrator is required to explain the tardiness. A building administrator will determine the validity of excuses and whether disciplinary action is required.
