SAP Referral Form
If the individual who you're referring poses an immediate risk to themselves or others, stop completing this form and contact a CHS administrator or school counselor (610-697-0111).  After you have established direct contact, please complete a SAP referral form. CASD employees are required to provide their name and email address to the team for internal communication purposes, but their identity in the referral process will remain confidential to the SAP team.
